Dear All:

On several occasions Kairos team members have needed medical care during a Kairos weekend or other Kairos event.

In such a situation it may be the effected person will not be able to provide medical information to fire rescue or other emergency or medical personnel.

For that reason we have prepared an optional/voluntary form that team members can fill out and give to the team leadership to have available in a medical emergency. Such a form could be very helpful if it listed drug allergies, diabetes, medication, heart history, etc. and includes the name and phone number of a contact person.

Again this is purely voluntary and the information should be kept in a sealed envelope until needed.
